Duration of Rotation:     4-8 weeks

Type of Rotation:     Ambulatory

Prerequisites:     Completion of Year II

Criteria for Passing:     Reasonable course attendance and participation and evaluation by faculty and staff.

Criteria for Honors:     Demonstrates a high degree of interest and proficiency and skill in participating with program and innovative case management.

Competencies:     Ability I: advanced level, Ability VII: intermediate level, Ability VIII: intermediate level, Ability IX: advanced level. Provisionally approved

Number of Hrs/Wk of Classroom/Discussion:        5-7

Number of Nights Oncall per Week:     None, unless requested

Planned Independent Projects for Students:     Submit an evaluation of his/her course involvement, and participate in care of a dying patient.

Description:      Involvement in patient care planning and case management of terminally ill patients at home under the Hospice Program.
