Types of Emphysema

Panacinar emphysema (left) affects lobules uniformly. In centriacinar emphysema (right) normal tissue separates the large emphysematous spaces from perilobular septa. Both forms can occur in smokers. Which form is worse in the upper lobes? Which form occurs in alpha-l-antitrypsin deficiency?
